  • Forward SAR form via Inter-office envelope to RCRMC Information Services Department mail stop #3890.
  • Upon receipt of your completed SAR form, your internal MISP HCRM access will be activated

In order to process your Account Request, we will need you to fill out the following information and complete the requested actions:
  • A signed copy of the eAccess Agreement faxed to (951)486-4815.
  • Completion of the form below. (Enter all required information)

Upon Review of your faxed eAccess Agreement your Account will become Activated.

Once Activated the following will occur:
  • You will receive an Email with your Temporary Password.
  • On your first login you will be required to change your password.
  • You will also be asked to change your password every 90 days in accordance with HIPAA regulations.
